Adamson Training the next generation of rhinoplastic surgeons requires time and dedication. A residency program designed to provide an active, hands on experience with enthusiastic attending surgeons is mandatory. This article examines some of the specific elements required to provide the optimal learning experience for surgical residents studying rhinoplasties. The Minimal Rhinoplasty 391 Ritchie A. L. Younger The minimal rhinoplasty involves conservative volume additions or subtrac¬ tions to effect a change in nasal appearance or function. Because rhinoplasty surgery is a flowing concert of surgical steps, the initial move irrevocably modifies the remaining movements. A series of flow algorithms are one surgeons intraoperative ongoing thought process in an attempt to organize and simplify this surgical symphony to achieve the perfect minimal rhino¬ plasty result. Skeletal Modifications in Rhinoplasty 413 Dean M. Toriumi and David A. Hecht Modifying the nasal skeleton during rhinoplasty to achieve predictable long term results remains one of the greatest challenges to the facial plastic surgeon. The bony nasal vault possesses unique anatomic features and aesthetic parameters that must be appreciated to optimize a favorable out¬ come. Special instrumentation and techniques are necessary to perform precise and effective manipulations to the bony vault while producing minimal surrounding soft tissue trauma. Changing the Nostril Shape 433 Anthony E. Brissett and David A. Sherris The nasal base and nostril shape are important characteristics to consider during the planning and execution of aesthetic and reconstructive rhinoplas ties. It is important to recognize that significant individual and racial/ethnic variations within the structural components of the nasal tip exist. When deciding on the appropriate surgical procedure to use for tip refinement and nostril shape augmentation, the structures that are responsible for creating a misshapen nasal tip and nostril must be considered. A systematic approach to the various subunits is described. Altering or augmenting one or all of these subunits will result in changes to the nasal tip and to the shape and orientation of the nostril. Nasal Valve and Nasal Alar Dysfunction 447 Robert W. Dolan Diagnosis and treatment of nasal valve and nasal alar dysfunction presents a challenge for even the most experienced clinician. This article describes the history, anatomy, diagnosis, and treatment of a variety of abnormalities involving the nasal valve and nasal ala. A classification of the abnormalities is emphasized with surgical treatment options specifically tailored to each category. Rhinoplasty and Sinonasal Disorders 465 Paul H. Toffel For decades, experienced pre endoscopic rhinologic surgeons have com¬ bined management of functional nasal disorders with cosmetic rhinoplasty in patients with indications and motivation. Now that endoscopic technique has become firmly established as part of the functional rhinologic armamen¬ tarium, it is logical that endoscopic nasal and sinus surgery could be com¬ bined with rhinoplasty in those patients who have functional and cosmetic indications. In a series of 2415 endoscopically managed functional nasal and sinus surgical patients over the past 13 years, the author performed rhinoplasty in 301 cases, or 12.5%, of the overall series. He found no diffi¬ culty in combining the modalities of functional endoscopic and traditional rhinoplastic surgery in selected patients. This article describes the author s experience with simultaneous and endoscopic sinonasal surgery and rhino¬ plasty. Managing the Tension Nose 479 Minas Constantinides and Jennifer Levine The tension nose is characterized by a high dorsal septum and an overpro jected tip held up by it. Rhinoplasty results can be unpredictable in these patients because the principles of Anderson s tripod concept do not al¬ ways apply. After taking down the high dorsum, tip support must be re¬ established in a predictable, reproducible fashion. Middle third support likewise requires re establishing a stable cartilage framework. Open septo rhinoplasty provides advantages over closed approaches in successfully negotiating through these challenging cases. The Mobile Tripod Technique: The Key to Nasal Tip Refinement 487 Alvin I. Glasgold, Mark J. Glasgold, and David B. Rosenberg The mobile tripod technique is a universal approach to nasal tip surgery. It provides excellent control for all types of tip modification, including narrowing, rotation, projection, and deprojection. This gives the rhinoplasty surgeon a secure method of dealing with any problems encountered in primary or revision rhinoplasty, including Caucasian, non Caucasian, and ethnic noses. Post Traumatic Rhinoplasty 503 Neal D. Goldman and Ramsey Alsarraf Post traumatic rhinoplasty consists of those surgical procedures that attempt to restore, reconstruct, and correct the nasal soft tissue, bony, and cartilagi¬ nous nasal elements following nasal injuries. Discussion includes treatment of simple abrasions, complex avulsions, acute and delayed nasal fractures, and major post traumatic structural and soft tissue deformities of the nose. Challenges of Secondary Rhinoplasty 515 Vito C. Quatela and Ivan Wayne Rhinoplasty is considered by many surgeons to be the most challenging procedure in facial plastic surgery. The rhinoplastic surgeon can experience the personal feelings of pride, satisfaction, humility, and frustration depend¬ ing on the outcome of his or her interventions. Cleft Rhinoplasty 539 Alyson Buckner and Craig A. Vander Kolk Rhinoplasty in the setting of the cleft lip continues to provide a challenge to the reconstructive surgeon. This article reviews the nasal anatomy associated with primary unilateral and bilateral cleft deformities and the nasal anatomy associated with the secondary deformity. The multitude of surgical tech¬ niques developed for repair and reconstruction of cleft lip nose is also presented and assimilated into a cogent paradigm for analysis and individu¬ alized surgical therapy for these problems. The Dissatisfied Patient 549 John M. Morehead Even after the best planned and executed rhinoplasties, some patients will be dissatisfied. Successful management of patient unhappiness is not only a vital part of surgical practice hygiene, it is also an opportunity to attend the patient, an aspect of medical practice that seems to be endangered. Ethical Considerations in Rhinoplasty 553 G. Richard Holt Rhinoplasty is a complex and complicated surgical procedure. Equally chal¬ lenging is the assessment and proper selection of candidates for the proce dure. The surgeon has a great responsibility to be diligent and insightful in the evaluation so that the patient s expectations may be fully discussed and a fair and honest discussion of the goals and risks be undertaken. Many ethical issues are associated with rhinoplasty, from marketing and advertising to inappropriate coding for insurance purposes.
