The management of anovulatory infertility in women with polycys¬
tic ovary syndrome (PCOS) has evolved significantly over the last
60 years. PCOS is now known to be a heterogeneous collection
of endocrine disturbances often involving hyperandrogenism and
insulin resistance. Treatments have become more sophisticated and
should target specific endocrinopathies. A comprehensive ap¬
proach toward inducing ovulation in these patients must take into
consideration the complexities of the disease and the risks and
benefits of a range of approaches. In this article, a historical perspec¬
tive of the disease is presented and a variety of methodologies
used to safely induce ovulation in patients with PCOS is discussed.

Several mechanisms exist that can reduce infertility through endo¬
metriosis, including impaired folliculogenesis. This article explores
the role of ovulation induction as a treatment of endometriosis
associated infertility. The use of adjuvant suppressive treatment
is also addressed. The success of in vitro fertilization in patients
with endometriosis is reviewed.

The Use of Ovulation Induction in the Treatment of
Unexplained Infertility 385

Unexplained infertility is a diagnosis of exclusion made after elimi¬
nating known causes of infertility in a standard evaluation. Despite
the lack of identifiable causes, empiric treatment is often successful.
Ovulation induction with clomiphene citrate or gonadotrophins
should be part of the initial therapy for this condition.
Ovarian Hyperstimulation Syndrome 399

Ovarian hyperstimulation syndrome often results from stimulation
with gonadotropins as a treatment for infertility. Strict monitoring
of E2 levels and follicular count should be maintained in an attempt
to predict its occurrence. Preventative measures, such as with¬
holding or substituting human chorionic gonadotropin with a
gonadotrophin releasing hormone agonist, withholding further
gonadotrophin therapy, administration of albumin, and, in cases
of in vitro fertilization, aspiration of ovarian follicles and cryopres
ervation may be beneficial. Therapy is supportive in nature with
surgical intervention reserved for those cases involving intraperito
neal hemorrhage or ovarian torsion.

Ample evidence demonstrates that fertility in women declines with
age, even after controlling for other causes of infertility. Because
women increasingly delay marriage and attempts at childbearing
within marriage, age related infertility has become a growing clini¬
cal problem. This article discusses the cellular and molecular mech¬
anisms of reproductive senescence, examining the role of mitochon¬
dria in cellular senescence through mutations in mitochondrial
DNA and the promotion of programmed cell death. The article
also discusses the roles of genomic instability, aneuploidy, and
telomeres in reproductive senescence.

In the past, the sole source of human luteinizing hormone and
human follicle stimulating hormone for clinical use was the urine
of postmenopausal women. Recombinant DNA technology has
now enabled direct production of these gonadotropins. This article

their success in clinical practice.

comparing the new recombinant gonadotropins for ovulation in¬
duction with the urinary products. Ovulation induction regimens
for patients who have typically been classified as hyperresponders

improve the response of women who are poor responders are pre¬
sented.

The escalating use of gonadotrophin ovulatory drugs and assisted
reproductive technology in the last two decades has resulted in a
dramatic upsurge of iatrogenic multifetal pregnancies and concom¬
itant complications. Multifetal pregnancy reduction (MFPR) was
introduced as a viable therapeutic option to circumvent the most
common complications associated with high order multifetal preg¬
nancies. MFPR is a reasonable and effective therapeutic option for
infertile couples faced with the enormous physical and psychologic
burden of carrying high order multifetal pregnancies.
Value of Superovulation Combined With
Intrauterine Insemination 481

Superovulation combined with intrauterine insemination (IUI) is
used for a variety of indications. For patients with unexplained
infertility, there is adequate evidence in the literature demon¬
strating an increase in pregnancy rates with the addition of IUI to
superovulation with gonadtropins. For patients with male factor
infertility, the addition of superovulation to IUI may improve preg¬
nancy rates. The cost effectiveness of these approaches needs to
be further evaluated.

This article illustrates how assisted reproduction techniques, al¬
though helpful to thousands of infertile couples, raise moral and
legal dilemmas created by conflicting moral principles that gener¬
ate conflicting demands. The goal is to alert the physician to some
of these dilemmas in the specific context of ovulation induction
and to illustrate how medical decisions in the context of assisted
production must often be guided by ethical considerations.
