He, Jiang Hsieh, and Hui Hu Progress in helical CT acquisition and processing over the last 5 years has lead to new applications and new requirements. Recent progress to meet these requirements is discussed in this article. The potential of future acquisi¬ tion developments and their potential to further import application are ex¬ plored. CT in Ischemic Stroke 515 Michael P. Marks Previously, CT was considered insensitive in the evaluation of the acute ischemic stroke patient; however, more recently detection of early CT find¬ ings has proved to be of prognostic value in the evaluation of these patients. The use of CT coupled with early acute phase therapy of stroke such as thrombolytic therapy has been shown to improve outcome in the acute stroke patient. CT of Head Trauma 525 Chi Shing Zee and John L. Go The advent of CT in the early 1970s revolutionized the diagnosis and man¬ agement of head trauma patients. CT remains to be the imaging modality of choice in the evaluation of patients with acute head trauma. High resolu¬ tion CT is excellent for evaluating facial and skull fractures. Neurosurgically significant lesions such as epidural hematomas, subdural hematomas, or depressed skull fractures are already detected by CT. The early detection of extra axial hematomas made possible by the increased availability of CT, results in early surgical interventions with marked improvement in morbidity and mortality in head trauma patients. Go and Chi Shing Zee Computed tomography remains the imaging modality of choice in the detec¬ tion of two processes: acute intracranial hemorrhage and calcification; how¬ ever, in the subacute and chronic stages of hemorrhage, MR imaging is more sensitive in aging and staging hemorrhage. FLAIR imaging increases the sensitivity of MR imaging in the detection of subarachnoid hemorrhage. The signal characteristics of calcification on MR imaging is variable, espe¬ cially on Tl weighted images decreasing its detectability. CT is superior to MR imaging in the detection of calcification. A number of factors including slice thickness, as well as window width and level may affect the detectabil¬ ity of calcification on CT. Contemporary Spinal CT Applications 559 Blake A. Tanenbaum Despite the widespread availability of MR imaging, CT remains a vital modality for the evaluation of disorders of the spine, offering superior assessment of bone abnormalities, as well as calcification. Advances in tech¬ nology have enhanced the capabilities of CT, providing exciting new options for data rendering and display. Depending on the clinical indication, either CT or MR may be the modality of choice in the evaluation of spinal disease. CT Angiography of the Intracranial Circulation 577 Pedro T. CT Angiography of the Carotid Artery 593 Pedro T. Vieco CT angiography shows promise as a noninvasive method to display carotid vascular pathology. This article discusses technique, stenosis measurements, pitfalls, strengths, and weaknesses of CT imaging of the carotid artery. Update on Sinonasal Imaging: Anatomy and Inflammatory Disease 607 Barbara Zeifer Functional endoscopic sinus surgery, now the standard of care for surgical treatment of chronic and recurrent sinusitis, has changed current concepts of sinus health and disease. Interpretation of the CT scan must reflect this focus on anatomy and function. This article seeks to provide a clear under¬ standing of sinonasal drainage. The normal frontal recess, ostiomeatal unit, and sphenoethmoid recess are considered along with the anatomic varia¬ tions that distort their appearance and predispose the patient to developing sinus disease. The spectrum of uncomplicated and complicated sinonasal inflammatory disease is discussed. Clinical and Surgical Application of Reformatted High Resolution CT of the Temporal Bone 631 Albert E. Alexander, Jr, Karen S. Caldemeyer, and Peter Rigby High resolution CT has played a pivotal role in temporal bone imaging since the early 1980s. Although the appearance of MR imaging technology Viii CONTENTS with its superior demonstration of fluid containing spaces of the inner ear and the posterior fossa structures, CT continues to offer distinct advantages for the evaluation of conductive hearing loss, and petrous apex lesions. With the advent of helical scanning techniques, CT is increasingly the im¬ aging study of choice for definitive preoperative temporal bone imaging. Orbital CT: Current Use in the MR Era 651 Menahem M. Maya and Linda A. Heier The additional advantages of spiral CT have further cemented CTs role as the screening examination of choice for the orbit. The shorter scan times permit ideal enhancement yet reduce the contrast load, providing the opti¬ mal postcontrast studies that routine imaging of the orbit requires. This article is not a comprehensive study of orbital CT, but a general review emphasizing CTs strengths, especially the evaluation of childhood leukoco ria (calcification), proptosis screening, and trauma.
