Massarotti Rheumatoid arthritis commonly affects the hand and wrist. The differen¬ tial diagnosis of inflammatory conditions affecting the hand is broad. A proper diagnostic approach necessitates a thoughtful interpretation of the presenting clinical features, laboratory tests, synovial fluid analysis, and radiographic data. Early, aggressive medical therapy with a combination of antiinflammatory and remittive agents may reduce mortality in selected patients and prevent the consequences of uncontrolled synovial prolifera¬ tion. Despite proper medical therapy, joint destruction often results and surgical treatment of the joint deformities may be required for restoration of function. The Wrist in Rheumatoid Arthritis 477 Jules S. Shapiro In the rheumatoid wrist, cartilage loss, synovial expansion with erosive disease and ligamentous laxity, result in loss of carpal height and stability and development in varying amounts of ulnar/palmar translocation and supination of the carpus on the distal forearm. This results in a painful wrist which influences deformity development distally. The aims of sur¬ gery are relief of pain and adequate stability while retaining as much motion as possible consistent with the degree of destruction present. A variety of procedures from soft tissue reconstruction to total arthrodesis are available to achieve these aims. A summary of the authors preferred techniques is presented. Metacarpophalangeal Joints in Rheumatoid Arthritis of the Hand 515 Craig R. Stirrat Metacarpophalangeal joint involvement is a well recognized problem in patients with rheumatoid arthritis. The joint destruction and deformity from the disease follow a progression of increasing digital volar subluxa tion and ulnar deviation. With early involvement, soft tissue procedures may be important in improving hand function and limiting progression. In late stage involvement, the silicone rubber implant arthroplasty devel¬ oped by Swanson is a valuable procedure for reconstruction. The outcomes of the implant procedure have been well studied and enable the hand surgeon to discuss with the patient the likely improvement following MP joint surgery. Finger Deformities in Rheumatoid Arthritis 531 Louis Rizio and Mark R. Belsky Swan neck and boutonniere deformities are the most common finger de¬ formities in rheumatoid arthritis. These deformities result from the effects of rheumatoid synovitis acting on the joints and the soft tissues that determine joint function. A thorough understanding of the digital exten¬ sor flexor mechanism is needed to appreciate the pathologic anatomy of the imbalance and the techniques available for reconstruction. The Rheumatoid Thumb 541 Andrew B. Stein and Andrew L. Terrono Involvement of the thumb is common in patients with rheumatoid arthri¬ tis. Six classic types of deformity have been described, but other patterns of deformity can also be encountered. Surgical treatment must be individu¬ alized for each patient and includes synovectomy, tenodesis, tendon trans¬ fer, arthrodesis, and arthroplasty. Extensor Tendon Problems in Rheumatoid Arthritis 551 Robert Lee Wilson and Michelle C. DeVito A prophylactic extensor tenosynovectomy can prevent tendon rupture and is indicated when the tenosynovitis does not respond to medical management or when an increase in size occurs. Tendon reconstruction can be achieved with either a tendon transfer or a tendon graft. A single extensor rupture is an indication for surgical treatment to prevent further tendon damage by eliminating the cause. Reconstruction of the wrist and metacarpal phalangeal joints may need to be performed before, during, or after treatment of the tendon problems, and careful planning is required. v} CONTENTS Rheumatoid Flexor Tenosynovitis and Rupture 561 Donald C. Ferlic Rheumatoid flexor tenosynovitis at the wrist can cause locking, limitation of motion, nerve compression, and rupture of tendons. To prevent and treat these conditions, a flexor tenosynovectomy, combined with nerve decompression and tendon reconstruction as indicated, is necessary. If tendons are ruptured, the flexor pollicis longus and profundus to the index finger are most commonly affected. On the fingers flexor tenosynovitis can also cause locking, decrease in range of motion and rupture tendons. Flexor tenosynovitis of the fingers should be treated with decompression of the tendons health with preservation of the pulley system. Juvenile Rheumatoid Arthritis 573 Barry P. Simmons, John T. Nutting, and Richard A. Bernstein Juvenile rheumatoid arthritis (JRA) affects up to 250,000 children in the United States, but its initial diagnosis is often complex and difficult. This article reviews presentation and management options. Recent studies have demonstrated success with various drug regimens. The authors also re¬ view the surgical approaches to the hand and wrist in JRA. Surgery of Systemic Lupus Erythematosus Arthritis of the Hand 591 Edward A. Nalebuff Systemic lupus erythematosus (SLE) is one of a group of three uncommon arthritic diseases, including psoriatic arthritis and sderodermn, th.it super¬ ficially resemble rheumatoid arthritis (RA). These diseases frequently in¬ volve the hand and wrist and therefore are significant to the hand surgeon. SLE has certain specific characteristics that separate it from the other two conditions. Each of these diseases has distinct features affecting the nature of the deformities as well as the treatment indicated to adequately care for these patients. Surgery of Psoriatic Arthritis of the Hand 603 Edward A. Nalebuff Psoriatic arthritis is an uncommon form of arthritis that poses specific problems for the hand surgeon. Although there are similarities between this form of arthritis and the more common rheumatoid or degenerative arthritis, this particular entity is unique, and physicians or surgeons treat¬ ing these patients should be aware of the differences to advise or carry out appropriate treatment. Before embarking upon specifics, it is fitting to review some of the fundamentals of this condition. An Approach to Complex Rheumatoid Hand and Wrist Problems 615 William F. Blair Each component of a strategy using multiple, concurrent operations should be clearly indicated. The strategy should be adopted only by surgeons experienced in each component of the reconstruction, ft is im¬ portant that elements in the postoperative hand therapy program not be CONTENTS Vli contradictory. An advantage of this strategy is that the total time devoted to surgical procedure, recovery, and rehabilitation for the patient is re¬ duced. Presumably, the cost of comprehensive procedures are reduced. When appropriately indicated by a hand surgeon experienced in all techni¬ cal components, a strategy of rheumatoid hand reconstruction using multi¬ ple, concurrent operations can be highly effective.
