The authors report the case of a 32-year-old man who was diagnosed with bilateral PAIS related to a prominent lateral tubercle of the talar posterior process, and who was treated by posterior arthroscopic excision through posterolateral and posteromedial portals in a prone position. His preoperative posterior ankle pain disappeared completely after surgery, and the patient returned to contact sports 3 months postoperatively.
