Zusammenfassung:OBJECTIVES. Revisions of hip endoprosthesis have been done more and more in the last few years in our hospital. Especially in cases of longer lasting loosening of the cup component great bone defects has to been treated. With the 'trabecular metal technology' new options for these cases are available. TMT is in its structure similar to the spongious bone; the bone incrow will be much better possible than in other materials. METHODS. We started using the modular TMT cups in 2004; until now more than 60 revision operations have been done. All these operations have been done in cases of bone defects according to Paprosky 2B and higher. In 5 cases also augmentation blocks from TMT has been used. In 28 cases the bone defects were build up with TMT and homologous bone from our bone bank. RESULTS. In all cases good primary stability of the cup component could be reached; all our patients have been mobilised after the operation with half weight bearing of the operated leg. Three months after the operation all patients have been painfree under full weight bearing. All the cases are under control and after 3, 6 and 12 months no migration and no loosening of the revision cup has been seen. CONCLUSIONS. With the TMT technology we have an new and very successful option in cases of cup revisions in hip surgery. We can built up also great bone defects and reach an good primary stability of the implant. Therefore good and fast post operative mobilisation is possible. After three months very good bony incrow of the implant can be achieved.
