Zusammenfassung: | In the cohorts analyzed by the authors, it is highly plausible that patients with more severe chronic obstructive pulmonary disease or higher ASA physical class were assigned to NA rather than GA cohort. [...]given the unbalance in patients' baseline characteristics, the question of whether the propensity score-matched analysis actually nullified the selection bias remains unanswered. To our evaluation, this point needs further clarification before we take the conclusion from this study to change our clinical practice, knowing clearly that use of NA in such surgery results in better pain relief and reduced postoperative complications as well as reduced costs associated with anesthesia [3,4]. [...]discharge times from hospital not only depend on recovery from anesthesia but also on several other perioperative and postoperative factors (i.e. operating room surgery schedule, length of surgery, postoperative pain, nausea-vomiting). |
