Zusammenfassung:Coronary calcifications remain associated to increased rate of complications and worse clinical outcomes, particularly in the setting of in-stent restenosis. We present the first reported case of combined rotational atherectomy and intravascular lithotripsy for the treatment of heavily calcified neoatherosclerosis. •Percutaneous coronary interventions of heavily calcified vessels are associated to poor procedural and clinical outcome•We present the first description of combined rotational atherectomy and intravascular lithotripsy in the setting of in-stent neoatherosclerosis•A combined approach using both intravascular lithotripsy and rotational atherectomy could enhance stent Expansion.
