Methods and Results: We studied the effects of EVAR on several outcome variables in 40 consecutive patients undergoing EVAR for abdominal and/or thoracic aneurysm with preserved ejection fraction. Echocardiography and brachial–ankle pulse wave velocity (baPWV) data were collected before, 1 week, and 1 year after EVAR. The changes in LVMI correlated with those in baPWV (R=0.32, P&lt;0.05). Among the 22 patients who were successfully followed up, 13 showed deterioration in exercise tolerance 1 year after EVAR. Diastolic wall strain, an index for LV distensibility, was lower at baseline in patients with worsening exercise tolerance than in those with unchanged tolerance. Conclusions: EVAR increased vascular stiffness and induced LV hypertrophy and diastolic dysfunction without a corresponding elevation of blood pressure in the acute and chronic phases. In addition, low LV distensibility at baseline was associated with the impairment of exercise tolerance. EVAR-induced stiffness of arteries leads to limited clinical symptoms.  (Circ J 2014; 78: 322–328)</description><identifier>ISSN: 1346-9843</identifier><identifier>EISSN: 1347-4820</identifier><identifier>DOI: 10.1253/circj.CJ-13-0877</identifier><identifier>PMID: 24292128</identifier><language>eng</language><publisher>Japan: The Japanese Circulation Society</publisher><subject>Aged ; Aged, 80 and over ; Angioplasty ; Aortic Aneurysm, Abdominal - pathology ; Aortic Aneurysm, Abdominal - physiopathology ; Aortic Aneurysm, Abdominal - therapy ; Aortic Aneurysm, Thoracic - physiopathology ; Aortic Aneurysm, Thoracic - therapy ; Blood Pressure ; Endovascular aortic repair ; Female ; Follow-Up Studies ; Heart Atria - pathology ; Heart Atria - physiopathology ; Humans ; Left ventricular diastolic function ; Male ; Vascular Stiffness ; Ventricular Function, Left</subject><ispartof>Circulation Journal, 2014, Vol.78(2), pp.322-328</ispartof><rights>2014 THE JAPANESE CIRCULATION SOCIETY</rights><lds50>peer_reviewed</lds50><oa>free_for_read</oa><woscitedreferencessubscribed>false</woscitedreferencessubscribed><citedby>FETCH-LOGICAL-c669t-8ea7634c0d1aadb02066e7fa018d26df1587d27e27a29e8ddcb98b0b23772ee43</citedby><cites>FETCH-LOGICAL-c669t-8ea7634c0d1aadb02066e7fa018d26df1587d27e27a29e8ddcb98b0b23772ee43</cites></display><links><openurl>$$Topenurl_article</openurl><openurlfulltext>$$Topenurlfull_article</openurlfulltext><thumbnail>$$Tsyndetics_thumb_exl</thumbnail><link.rule.ids>314,776,780,1877,4010,27900,27901,27902</link.rule.ids><backlink>$$Uhttps://www.ncbi.nlm.nih.gov/pubmed/24292128$$D View this record in MEDLINE/PubMed$$Hfree_for_read</backlink></links><search><creatorcontrib>Takeda, Yasuharu</creatorcontrib><creatorcontrib>Sakata, Yasushi</creatorcontrib><creatorcontrib>Ohtani, Tomohito</creatorcontrib><creatorcontrib>Tamaki, Shunsuke</creatorcontrib><creatorcontrib>Omori, Yosuke</creatorcontrib><creatorcontrib>Tsukamoto, Yasumasa</creatorcontrib><creatorcontrib>Aizawa, Yoshihiro</creatorcontrib><creatorcontrib>Shimamura, Kazuo</creatorcontrib><creatorcontrib>Shirakawa, Yukitoshi</creatorcontrib><creatorcontrib>Kuratani, Toru</creatorcontrib><creatorcontrib>Sawa, Yoshiki</creatorcontrib><creatorcontrib>Yamamoto, Kazuhiro</creatorcontrib><creatorcontrib>Mano, Toshiaki</creatorcontrib><creatorcontrib>Komuro, Issei</creatorcontrib><title>Endovascular Aortic Repair Increases Vascular Stiffness and Alters Cardiac Structure and Function</title><title>Circulation Journal</title><addtitle>Circ J</addtitle><description>Background: Endovascular aortic repair (EVAR) is performed in patients with thoracic or abdominal aortic aneurysm because it is less invasive than conventional open repair.
