갈색세포종은 고혈압, 두통, 심계항진 등의 다양한 임상 양상을 나타내는 드문 종양이나 드물게 갈색세포종의 급작스런 파열이나 괴사 시에는 다량의 카테콜라민 방출에 따른 비특이적인 심부전, 급성 복증, 쇼크 등이 나타나 치명적인 손상이 발생한다. 저자들은 심인성 쇼크로 발현된 갈색세포종 환자를 경험하였기에 원인 미상의 심부전, 급성 복증이나 쇼크 등의 증상을 보이는 환자에서 갈색세포종을 감별진단으로 고려해 볼 필요가 있다고 생각되어 문헌 고찰과 함께 보고하는 바이다. Pheochromocytoma is derived from the chromaffi n cells and patients with pheochromocytoma present with several signs and symptoms by producing, storing and secreting catecholamine. Spontaneous rupture or necrosis of pheochromocytoma is extremely rare, but it can be lethal because of the dramatic change in the circulation such as an acute abdominal emergency or shock. Spontaneous remission of the clinical symptoms due to necrosis of the pheochromocytoma is rare. We describe such a case that presented with cardiogenic shock due to extensive necrosis of the pheochromocytoma and this was followed by spontaneous remission of the clinical symptoms without removal of the pheochromocytoma.
