Zusammenfassung: | Introduction: Neuroblastoma is a malignant neoplasm that affects sympathetic nerve cells, mainly in the adrenal medulla. This recurrent pathology in childhood presents challenges in terms of both early detection and therapeutic management. This study describes the case of a patient who was diagnosed with congenital neuroblastoma in the first month of life, treated surgically and with a favorable evolution to date. Objective: To report the case of a patient with congenital neuroblastoma, followed by a narrative review of the literature, with emphasis on describing the diagnostic process, therapeutic options and outpatient follow-up. Method: This study consists of a case report and a narrative literature review. Data was collected by interviewing the family, applying an online form and analyzing the laboratory and imaging results used for diagnosis. The scientific articles selected for analysis were published between 2018 and 2024. Case report: child, currently 2 years old, diagnosed with right adrenal tumor, confirmed congenital neuroblastoma after surgical resection and histopathological analysis. The cystic mass was detected on a post-natal urinary tract ultrasound, which was carried out due to dilation in the left renal pelvis identified on a 2nd trimester morphological ultrasound. In view of the rapid increase in the size of the lesion, 40% in 15 days, surgical resection was chosen. Diagnostic staging of the neuroblastoma indicated regular follow-up until 10 years of age. Conclusion: Early diagnosis and multidisciplinary management of congenital neuroblastoma are crucial for successful treatment. Complete removal of the lesion led to patient stability and adequate development without additional interventions. Keywords: Neuroblastoma; Adrenal Cancer; CNS neoplasm. Introducao: neuroblastoma e uma neoplasia maligna que afeta celulas nervosas simpaticas, principalmente na medula adrenal. Esta patologia recorrente na infancia, apresenta desafios tanto em relacao a deteccao precoce quanto ao manejo terapeutico. O presente estudo descreve o caso de um paciente que foi diagnosticado com neuroblastoma congenito no 1 mes de vida, tratamento cirurgico e evolucao favoravel ate o momento. Objetivo: relatar o caso de um paciente com neuroblastoma congenito, seguido de uma revisao narrativa da literatura, com enfase na descricao do processo diagnostico, opcoes terapeuticas e seguimento ambulatorial.
