PURPOSE:To read information on each splitted original plane, by providing zone plates in response to each region of the original plane, and providing a light shield means between the zone plates, and forming the information of each original plane on a sensor. CONSTITUTION:Thin film sensors 171-173 are provided on a transparent base 19, and zone plates 151-153 are provided on a transparent base 18. The reference pattern for alignment obtained from the same mask is provided on the transparent based 19, 18 with photo etching, and the alignment of the thin film sensors and the zone plates is made based on the reference pattern. Light shield means 201-204 are provided by implanting a part of the base 18 having the zone plates 151-153, and the incidence of the information other than the information to be read in via the adjacent zone plates can be prevented. Split information 141-143 on an original 13 is formed on the thin film sensors 171- 173 via the zone plates 151-153 for read-in.
