Zusammenfassung:	PROBLEM TO BE SOLVED: To make it possible to store a certificate of delivery even in a management department by providing a recipient, two certificates of delivery having acceptance code fill-in parts, and one certificate of addressee having an address and an acceptance code fill-in part aligned at the same surface side on a front surface side, and providing releasable and hard releasable adhesive structure at a delivery vessel at the rear surface sides of the certificates of delivery and addressee. SOLUTION: A certificate of delivery, duplicates 2, 3 and a certificate 4 of addressee are connected, the certificate 4 is partitioned with address, acceptance code fill-in pieces 4a, 4b, and recipient, acceptance code fill-in columns 10 to 13 are provided at the certificate of delivery and the duplicates 2, 3. A receipt seal impression column 16 is provided on the front surface side of the certificate 2. The adhesive structure that the one surface of an intermediate sheet 2 is releasably adhered with releasable adhesive 21 after adherence to an envelope and adhesive 23 difficult to release after adherence is covered with release sheet 24 on the other surface of the sheet 22 is provided on the rear surface side of each of the certificate of delivery and the duplicates 2, 3, and the sheet 22, the adhesive 23 and the sheet 24 are so provided as to affect to the rear surface of the certificate 4. After the receipt seal is impressed, the certificate of delivery and the duplicates 2, 3 can be easily released from the sheet 2.
