The bent parts 14a and 14b are connected through connecting members 16 so that they can be bent. The paired bent PC boards 14 and 14 are joined at an appropriate interval defined by separators 18, and the interval between the PC boards is filled with concrete. Then, the foundation form 10 is made integral with the concrete, and thereby installation of plywood forms or the like that are removed afterward becomes unnecessary.
