PROBLEM TO BE SOLVED: To provide a plate-shaped member for forming a suction tool which enables a manufacturing process for forming a check valve to be simplified, and of which the manufacturing cost can be suppressed to be low.SOLUTION: The plate-shaped member includes: a base for forming a cylindrical body (30) by bonding end parts with each other in a specific direction; and a check valve forming piece extended from the prescribed part of the base, for forming a check valve (50) by being bent. The base includes: a base bonding portion; a connection part forming portion for forming a connection part (30b); and a suction portion forming portion including a check valve forming piece connection part, for forming a suction portion (30c). The check valve forming piece has a shape enabling the check valve forming piece to move between an allowing position for allowing suction by being brought down inward with a check valve bend portion (50c) as a fulcrum and a closing position for closing the suction portion (30c) by being raised with the check valve bend portion (50c) as the fulcrum.
