There are formed a side face positioning structure body (insertion guide groove) 39 for positioning both side faces of the optical fiber 2 and an end positioning structure body (stopper part) 40 which positions by abutting and stopping the end of the optical fiber 2. A third substrate 42 is provided which pinches the optical fiber 2 except the end part thereof between the third substrate 42 and the other face 35 of the second substrate 34 and fixes the optical fiber 2 with an adhesive.
