At least two bone screws or pins 3 can be fixed between the jaw 35 and the surface 47 of a clamp positioned at the upper part of the upper swivel element 4. The upper swivel element 4 can be rotated about a pivot shaft 60 relative to the middle swivel element 5. The pivot shaft extending perpendicular to the axis is fixed on a first front face 61 of the middle swivel element 5 positioned to cross the axis 38.
