Zusammenfassung:PROBLEM TO BE SOLVED: To provide an information-concealing form which can be opened easily without causing dust and can be re-concealed easily when returned. SOLUTION: One side edge facing the binding side edge 5 of a lower layer paper sheet 3 is projected outside of an upper layer paper sheet 2 to be an adhesive side edge 7, and an easy bending line 16 in parallel with the binding side edge is formed in the lower layer paper sheet. The mutual portion side edges, which are located further closer to the binding edge side than to the easy bending line of the two side edges orthogonal with the easy bending line in the upper layer paper sheet and the lower layer paper sheet, are un-peelably bonded to be a strong bonding portion 17. On the other hand, the mutual portion side edges located further closer to the adhesive edge side than to the easy binding line and the mutual side edges of the upper layer paper sheet and the lower layer paper sheet each located inside of the adhesive side edge in the two side edges are peelably bonded to be a weak bonding portion 18. The weak bonding portion can be peeled when opening the information-concealing form, and the upper layer paper sheet and the lower layer paper sheet can be valley-folded at the easy bending line when re-sealing the form, allowing the adhesive side edge of the lower layer paper sheet to be bonded to the facing surface of the upper layer paper sheet through an adhesive layer.
